How to apply
- Applications will only be considered from NHS staff. Direct applications are not accepted.
- Applicants must be receiving treatment for Diabetes from a hospital in the UK.
- Age limit normally 19 years old.
- Applicants must be resident in the UK.
- The applicant's family must be in receipt of means tested benefits (e.g. Universal Credit, JSA, ESA, or Income Support). Disability Living Allowance and Carers Allowance are not relevant.
- Grants will not be available as a regular supplement to income and should only be applied for in times of crisis.

Applications must be submitted by an NHS nurse or doctor. Enter your NHS email address below and we will send you a secure one-time link to the referral form. The link can only be used for one submission.
